Mechanism and Usage
Triamterene is a potassium-sparing (antikaliuretic) water pill combining the two agents—Hydrochlorothiazide and Triamterene. The first of them, Hydrochlorothiazide lowers blood pressure and has a diuretic effect while preventing absorption of salt. In particular, renal tubular absorption of sodium and chloride ions is stopped.
The onset and mechanism of Triamterene is similar to Hydrochlorothiazide. As a diuretic, it has a natriuretic potency. It blocks the uptake of sodium on the lumen side of the kidney collecting tubule in exchange for potassium and hydrogen. Due to its action, the exceeding hydrogen and potassium loss caused by hydrochlorothiazide is prevented, sodium excretion grows.
The effect of potassium accumulation is reported in patients with Addison's disease (without aldosterone). Triamterene cannot be considered a viable alternative to mineralocorticoids.
Dosage and Taking

Skipped Dose
In case you failed to take Triamterene while taking it regularly, do it as soon as you will be able to. If it is nearly the time for the next dose, just forget it and keep to your regular scheme. Double dose is not recommended.
Storage recommendations
Room temperature is recommended for storing the medicine (20–25 degrees C, or 68–77 degrees F) and a lightproof tight box. Protect Triamterene from humidity, high temperatures, and light. Store it away from children and pets. Bathroom is not a proper place to keep Triamterene.
Safety information
Triamterene should not be taken in case of allergic reactions to any of its ingredients. Avoid potassium supplements or potassium containing products, aldosterone blockers including eplerenone, or other diuretics including amiloride, while you are taking the medicine. Stop taking Triamterene in case of urination problems, increased blood potassium level, severe liver or kidney disease.
